Job Summary
Under the direction of the Manager, Home & Community Care, the position uses independence and initiative to plan, organize, implement and evaluate professional nursing care for clients of all ages with chronic, acute, palliative or rehabilitative health care needs in community settings, services include assessment, direct medical-surgical nursing care, case management, health teaching, support consultation, liaison and referral in a case management context, ensures that the model of client self care and independence is maintained; participates in the administration of all Acts and Regulations applicable to nursing functions, may supervise ancillary staff.
Qualifications
Education, Training and Experience
• Graduate from a recognized school of nursing plus three (3) years experience in community home care or acute care nursing.
• Current and active registration with the College of Registered Nurses of British Columbia (CRNBC) required.
• Preferably related experience in acute care nursing and/or community health nursing.
• Knowledge of and skill in acute care nursing and community nursing.
Demonstrated Skills and Abilities
• Ability to adapt clinical and professional skills to the home and community settings.
• Valid drivers licence.
• Problem solving skills.
• Considerable judgement in professional nursing care.
• Travel is a requirement of this position.
• Transportation arrangements must meet operational requirements.
• Other abilities as per job description.
